Analysing the implementation of infection prevention and control measures in health care facilities during the COVID-19 pandemic in the African Region

BackgroundThe declaration of SARS-CoV-2 as a public health emergency of international concern in January 2020 prompted the need to strengthen infection prevention and control (IPC) capacities within health care facilities (HCF). IPC guidelines, with standard and transmission-based precautions to be put in place to prevent the spread of SARS-CoV-2 at these HCFs were developed. Based on these IPC guidelines, a rapid assessment scorecard tool, with 14 components, to enhance assessment and improvement of IPC measures at HCFs was developed. This study assessed the level of implementation of the IPC measures in HCFs across the African Region during the COVID-19 pandemic.MethodAn observational study was conducted from April 2020 to November 2022 in 17 countries in the African Region to monitor the progress made in implementing IPC standard and transmission-based precautions in primary-, secondary- and tertiary-level HCFs. A total of 5168 primary, secondary and tertiary HCFs were assessed. The HCFs were assessed and scored each component of the tool. Statistical analyses were done using R (version 4.2.0).ResultsA total of 11 564 assessments were conducted in 5153 HCFs, giving an average of 2.2 assessments per HCF. The baseline median score for the facility assessments was 60.2%. Tertiary HCFs and those dedicated to COVID-19 patients had the highest IPC scores. Tertiary-level HCFs had a median score of 70%, secondary-level HCFs 62.3% and primary-level HCFs 56.8%. HCFs dedicated to COVID-19 patients had the highest scores, with a median of 68.2%, followed by the mixed facilities that attended to both COVID-19 and non-COVID-19 patients, with 64.84%. On the components, there was a strong correlation between high IPC assessment scores and the presence of IPC focal points in HCFs, the availability of IPC guidelines in HCFs and HCFs that had all their health workers trained in basic IPC.ConclusionIn conclusion, a functional IPC programme with a dedicated focal person is a prerequisite for implementing improved IPC measures at the HCF level. In the absence of an epidemic, the general IPC standards in HCFs are low, as evidenced by the low scores in the non-COVID-19 treatment centres.

Management and control of an outbreak of NDM-producing Klebsiella pneumoniae in a teaching hospital

Abstract Background Outbreaks caused by New Delhi metallo-β-lactamase (NDM)-producing Enterobacteriaceae are a major public health threat. We report an outbreak caused by NDM-producing Klebsiella pneumoniae (KP) in an Italian teaching hospital and describe the composite management approach that led to a successful containment. Methods On August 12 2022, a 39-year old Ukrainian patient with known colonization (rectal swab) by NDM-producing KP was admitted to the Hematology Unit of the University Hospital of Verona, Italy. Six days after, a second patient tested positive for carbapenem-resistant (CR) KP from rectal swab and 2 additional cases (rectal and pharyngeal swab respectively) were identified in the following 2 weeks. The 4 cases were identical strains. Results A multidisciplinary team coordinated the response actions. Isolation measures were ordered for positive patients, including use of dedicated instruments. Screening at admission and every 7 days was placed for all the ward patients. On-site inspections and microbiological screening of ward surfaces and devices were carried out. Infection prevention and control (IPC) measures were strengthened among personnel. Environmental cleaning was implemented by deep treatment of critical points (e.g. toilets) and reinforcement of routine cleaning. The Risk Manager developed an original checklist based on literature to review the implementation of IPC practices for CR Enterobacteriaceae. No new cases were identified from screening in the following weeks and the Unit returned to standard IPC measures. Given that environmental samples were negative and the cases had overlapping personnel, the hypothesis of contact transmission through caregivers was the most plausible. Reinforcement of training on hand hygiene and transmission-based precautions was planned. Conclusions It was possible to contain the hospital outbreak through rapid and diversified reaction capacity. Continuous IPC education is an essential control measure. Key messages • The spread of NDM-producing Enterobacteriaceae in the hospital setting must be countered with enhanced prevention and, in case of outbreak, rapid control. • Effective containment involves a mix of established measures (screening, hygiene, environmental controls) and innovative approaches (e.g. literature-based checklists).

Lassa Fever Infection among Healthcare Workers during the 2018 Outbreak in Nigeria

Introduction: Healthcare workers (HCWs) are potentially exposed to infection during viral hemorrhagic fever outbreaks. In the wake of 2018, Nigeria experienced an unprecedented surge in cases of Lassa fever (LF), which affected HCWs. To guide infection prevention and control (IPC) strategies in similar settings, we characterize HCWs' infection and describe the gaps in IPC standards and practices during the outbreak. Methods: Data was collected using a structured questionnaire, interview, and review of case notes of 21 HCW with laboratory-confirmed Lassa fever who were treated at the Irrua Specialist Teaching Hospital (ISTH) Irrua and the Alex-Ekwemen Federal Teaching Hospital, Abakaliki (AEFETHA), between 1st January and 27th May 2018. Information collected was the patients' socio-demographic characteristics, date of potential exposure and onset of illness, nature, and type of exposure, clinical features, outcome, use of personal protective equipment (PPE), and personnel IPC training and were analyzed using descriptive statistics with Microsoft Excel. Results: The study included 21 HCWs, and 12 (57.14%) were doctors. The case fatality rate was 23%. Nearly two-thirds (62%) of the HCWs could describe a likely procedure leading to their exposure and infection. Among 13 HCWs, 85% had multiple blood and body fluids exposure, while 15% had needle stick injury or scalpel cut. About one-fifth of the participants had received some IPC training. Conclusion: Limited IPC adherence and inappropriate risk assessment were identified as factors leading to Lassa fever exposure and infection among HCWs. There is an urgent need to provide IPC training for all HCWs and to ensure an adequate supply of IPC materials to all healthcare facilities as part of emergency preparedness, especially in LF endemic areas.

Adherence to infection prevention and control measures and risk of exposure among health‐care workers: A cross‐sectional study from the early period of COVID‐19 pandemic in Addis Ababa, Ethiopia

AbstractBackground and AimHealthcare workers (HCWs) are considered a high‐risk group for severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) exposure, ascribed to the amount of time they spend in health‐care facilities (HCFs). This study aimed to assess HCWs' compliance with Infection Prevention and Control (IPC) procedures and the risk of exposure during the early period of the pandemic in Addis Ababa, Ethiopia.MethodsA descriptive cross‐sectional survey was conducted from June to September 2020. With a response rate of 79.2%, a standardized questionnaire was administered among 247 HCWs, working in eight HCFs. Descriptive and multivariate regression analysis was carried out in STATA version 16.ResultsAbout 22.5% (55) of the HCWs had proper adherence to IPC procedures. Of the total participants, 28.2% (69) had proper use of Personal Protective Equipment (PPE), 40% (98) had proper hand hygiene practices, and 33.1% (81) had frequently cleaned their working environment. HCWs who received training on IPC protocols were four times more likely to follow IPC standards than those with no training (adjusted odds ratio [AOR] = 3.93; 95% confidence interval [CI]: 1.46, 10.58). Besides, HCWs working in treatment centers were four times more likely to follow IPC standards than those working in conventional hospitals (AOR = 3.61; 95% CI:1.63, 8.02). Nurses were four times more likely to have adherence to IPC measures than cleaners and runners (AOR = 4.37; 95% CI: 1.38–13.88).ConclusionThe nature and magnitude of the pandemic did not introduce the required degree of adherence to IPC procedures, per se does not match the level of diligence needed to halt SARS‐CoV‐2 transmission. Our finding suggested that providing periodic training of HCWs with particular emphasis on nonclinical staff is commendable. Furthermore, it is necessary to maintain resilent IPC in HCF through continous follow up and safety drills, to assess the readiness of HFCs' adherance to IPC measures under normal circumstances, which could improve prepardeness for an effective response during epidemics.

Assessing implementation of prevention and infection control programs in armenian hospitals during the COVID-19 pandemic

Aim of the study was to assess the current state of implementating key aspects of infection prevention and control (IPC) in Armenian hospitals, defined by the main WHO components, using the survey tool IPCAF (Infection Prevention and Control Assessment Framework). Materials and methods. 113 hospitals were involved in the study. All participants filled out the IPCAF questionnaire, consisting of 8 sections (each rated up to 100 points) devoted to various IPC aspects. Depending on the total final score, IPC programs were divided into the following categories: advanced (601800 points), intermediate (401600 points), basic (201400 points) and inadequate (0200 points). The study conducted a descriptive analysis of the overall IPCAF score as well as score evaluation of the relevant core components and some selected questions of particular interest. Results. The 2021 total average score for all IPCAF components for all hospitals in Armenia is 578.07.9 points, which is currently regarded as an intermediate level. At the same time, 47 (41.6%) hospitals were assigned to the advanced category, 63 (55.7%) to the average category and 3 (2.7%) to the basic category. Analysis of the main IPC components (CC, Core component) revealed that the maximum average scores were obtained for the components CC2 (IPC Guidelines) 80.8 points, CC8 (Creating a safe environment in medical premises, as well as materials and equipment for IPC) 75.5 points and CC7 (Workload, staffing and number of beds) 75.2 points. The lowest average scores were obtained for the key components: CC4 (Surveillance for healthcare-associated infections) with 54.7 points and CC3 (IPC education and training) with 59.5 points. Conclusion. Legislative and practical changes implemented in the real world during the novel coronavirus infection (COVID-19) pandemic have significantly contributed to improved scores for many key components of IPC programs. The study found that the IPCAF is a useful tool for assessing IPC standards and identifying gaps, regardless of a country economic development level. In our opinion, the re-use of IPCAF in all medical institutions should be encouraged, which is important for monitoring changes and trends in IPC, as well as proposing individual strategies in the development and improvement of IPC.

Challenges to infection control in early communication intervention: A scoping review.

BackgroundPersonal protective equipment (PPE) and infection prevention and control (IPC) measures are crucial to preventing the spread of coronavirus disease 2019 (COVID-19). This study used a scoping review to investigate the challenges that exist when speech–language therapists (SLTs) use IPC measures for providing early communication intervention (ECI).ObjectivesTo describe existing, recent literature on PPE and IPC measures used in early intervention through a scoping review (steps 1–5) and to consult local clinicians to investigate how SLTs who provide ECI in South Africa relate to these findings (step 6 of the scoping review).MethodA scoping review was performed which followed the PRISMA-ScR framework. Because of limited literature on PPE and IPC measures used by SLTs in providing ECI, the inclusion criteria were adjusted to include PPE and IPC measures used by healthcare workers (HCWs) who provide early intervention to the population of infants and toddlers up to 3 years old. At the time of the review, articles were not older than 10 years and were published between 2011 and 2020. The scoping review included a consultation with South African SLTs who provide ECI, including during the COVID-19 pandemic. A pilot study was conducted prior to the consultations. Seventeen clinicians were included in total. Data from both the pilot study and main consultation were transcribed and analysed in the results using thematic analysis.ResultsFourteen articles were included in the study. The scoping review of existing literature identified challenges to implementing IPC measures, namely the care and behaviour of young children, infrastructure and system challenges, poor compliance and lack of training and a lack of standard IPC protocols. Clinicians in the consultation phase confirmed these challenges and reported that IPC measures did not consider ECI populations nor the settings in which services were provided. Suggestions from the literature for improved infection control included hand hygiene, improved supplies and infrastructure and education and training. Clinicians in the consultation added practical suggestions for implementing IPC measures within ECI, which included an increase in parent-led intervention as well as cleaning and disinfection strategies.ConclusionThis study identified challenges and recommendations of SLTs who use PPE and IPC measures whilst providing ECI. Understanding these challenges can benefit ECI services and future research efforts focused on improving ECI services whilst maintaining IPC standards.

Infection Prevention Strategies Concordance in Canadian Dental Schools During the COVID-19 Pandemic

ObjectiveThe aim of this research was to identify variation in specific infection prevention and control (IPC) strategies across all dental schools in Canada and to evaluate the concordance concerning COVID-19 pandemic-related IPC strategies reported by clinic directors or IPC officers (CDs/IPCOs) and those reported by students, staff, and faculty in the schools.MethodA cross-sectional analysis within a prospective cohort study. Participants in the cohort study reported IPC strategies used in their schools during April or May 2021. Independently, CDs/IPCOs reported IPC strategies in school protocols in July 2021.ResultsOf the 600 participants recruited, 332 participants who were involved in the provision of in-person dental care were further analysed. Of the 16 IPC strategies investigated, only 3 were reported by CDs/IPCOs to be used at all schools, and another 8 strategies were used by 8 or 9 of 10 or by 1 of 10 schools, indicating that concordance across schools was good for 11 of 16 strategies. Agreement between study participants and the CDs/IPCOs varied considerably by strategy (ranging between 50% and 100%) and by school (ranging between 42.9% and 97.2%). The strategies with the highest mean agreement percentage across schools were “screening or interviewing patients before appointment for COVID-19–related symptoms” (92.7%) and “checking the temperature of the staff members at least once a day using a thermometer” (91.5%).ConclusionsThe level of agreement in the use of strategies between participants working in clinics and CDs/IPCOs varied considerably by strategy and by school. Given the low COVID-19 infection rates in dental schools and the reported differences in IPC protocols, key strategies should be identified. During the pandemic, IPC protocols in Canadian dental schools evolved rapidly. Comparing different strategies might help develop a unified standard IPC protocol.

Compliance of dental health care providers with the infection prevention and control protocol in Gaza Strip: a cross-sectional study

BackgroundThe Palestinian Ministry of Health (MOH) developed the infection prevention and control (IPC) protocol in 2004 to control infection among dental health care providers in their interactions with clients, the community, and the environment. Adherence to the protocol has not previously been reported. We compared compliance of dental health-care providers with the IPC protocol and in MOH and United Nations Relief and Work Agency for Palestine Refugees (UNRWA) clinics and assessed factors affecting adherence. MethodsWe did a cross-sectional observational study in all Gaza Strip governorates. A self-administered questionnaire was sent to 137 dental health-care providers in the region to collect data about compliance with the IPC protocol. In parallel, we completed two observational checklists for each dental health-care provider during visits to clinics: one assessing compliance (completed three times with 3-day intervals) and one assessing the presence and availability of IPC systems and supplies (completed once). Data were analysed by χ2 and t test, with p<0·05 indicating significance. Ethics approval was obtained rom the Palestinian Helsinki Committee, and dental health-care providers gave written informed consent for inclusion in the study. Findings137 (100%) dental health-care providers completed questionnaires. 82 (60%) respondents were men and 55 (40%) were women. 90 (66%) worked in MOH clinics (67 dentists and 30 practical nurses) and 40 (34%) in UNRWA clinics (24 dentists, 14 practical nurses, one dental nurse, and one oral hygienist). 134 (98%) of 137 dental health-care providers had received a hepatitis B vaccination. 97 (80%) dental health-care providers in MOH clinics and 38 (95%) of those in UNRWA clinics showed compliance in applying the IPC protocol during practice (p<0·001). Only 40 (29%) of respondents in MOH clinics and only four (3%) in UNRWA clinics had a hard copy of the IPC protocol. 29 (21%) of 137 respondents had systems for monitoring infection rates in their clinics. 65 (47%) attributed the shortage of IPC materials in clinics, such as bleaching agents, suction tubes, face shield or masks, and aprons, to lack of availability in central medical stores. 104 (76%) of respondents showed good knowledge of the importance of IPC standards for society, 115 (84%) had suggestions for modification of the protocol, and 137 (100%) reported that there was no protocol for prevention of medical injuries (eg, needlestick injury) as only a post-exposure programme was included in the IPC protocol. Adherence of dental health care providers to the IPC protocol was significantly higher in UNRWA clinics than in MOH clinics (p<0·001). InterpretationThere is an urgent need to develop the IPC protocol for dentistry. Future interventions and training, securing essential IPC materials, and increased monitoring of IPC coverage in MOH and UNRWA clinics will be needed to achieve this goal. FundingNone.

Massive Spread of OXA-48 Carbapenemase-Producing Enterobacteriaceae in the Environment of a Swiss Companion Animal Clinic.

Background: Companion animal clinics contribute to the spread of antimicrobial resistant microorganisms (ARM) and outbreaks with ARM of public health concern have been described. Methods: As part of a project to assess infection prevention and control (IPC) standards in companion animal clinics in Switzerland, a total of 200 swabs from surfaces and 20 hand swabs from employees were collected during four days in a medium-sized clinic and analyzed for extended spectrum beta-lactamase-producing Enterobacteriaceae (ESBL-E), carbapenemase-producing Enterobacteriaceae (CPE), vancomycin-resistant enterococci (VRE), and methicillin-resistant staphylococci (MRS). Results: A total of 22 (11.0%) environmental specimen yielded CPE, 14 (7.0%) ESBL-E, and 7 (3.5%) MRS; MR Staphylococcus aureus were isolated from two (10.0%) hand swabs. The CPE isolates comprised Escherichia coli, Klebsiella pneumoniae, Enterobacter hormaechei, Citrobacter braakii, and Serratia marcescens. Whole genome sequencing revealed that all CPE carried closely related blaOXA-48 plasmids, suggesting a plasmidic spread within the clinic. The clinic exhibited major deficits in surface disinfection, hand hygiene infrastructure, and hand hygiene compliance. CPE were present in various areas, including those without patient contact. The study documented plasmidic dissemination of blaOXA-48 in a companion animal clinic with low IPC standards. This poses a worrisome threat to public health and highlights the need to foster IPC standards in veterinary clinics to prevent the spread of ARM into the community.

Risk Factors Associated with Nursing Home COVID-19 Outbreaks: A Retrospective Cohort Study.

Background: The coronavirus disease 2019 (COVID-19) pandemic had a devastating impact on nursing homes/long-term care facilities. This study examined the relationship between geography, size, design, organizational characteristics, and implementation of infection prevention and control (IPC) measures and the extent of COVID-19 outbreaks in nursing homes in the Autonomous Province of Trento (Italy) during the time frame of March-May 2020. Methods: The analysis included 57 nursing homes (5145 beds). The association between median cumulative incidence of COVID-19 cases among residents and characteristics of nursing homes was assessed by Mann–Whitney U test, Kruskal–Wallis test or Spearman rho. To evaluate the potential confounding of geographical area, a 2-level random intercept logistic model was fitted, with level 1 units (patients in nursing homes) nested into level 2 units (nursing homes), and “being a COVID-19 case” as the dependent variable. Results: Median cumulative incidence was not significantly associated with any of the variables, except for geographical region (p = 0.002). COVID-19 cases clustered in the part of the province bordering the Italian region most affected by the pandemic (Lombardy) (45.2% median cumulative incidence). Conclusions: Structural/organizational factors and standard IPC measures may not predict the epidemiology of COVID-19 outbreaks and be sufficient alone to protect nursing homes against them.

A public health response to a newly diagnosed case of hepatitis C associated with lapse in Infection Prevention and Control practices in a dental setting in Ontario, Canada.

Haliburton, Kawartha, Pine Ridge District Health Unit (HKPRDHU) investigated an exposure in an Ontario operatory dental facility related to a newly diagnosed hepatitis C virus (HCV) infection caused by a virus with an uncommon hepatitis C genotype. Lapses in Infection Prevention and Control (IPAC) and a second epidemiologically-linked case (with the same uncommon hepatitis C genotype) were identified, prompting a broader public health response and outbreak investigation. a) To describe the investigation of a newly diagnosed case of hepatitis C; b) to describe the broader public health response, and c) to address a paucity in the literature related to the risk of disease transmission in dental settings due to IPAC lapses. A collaborative approach with two dental practices, public health partners and regulatory bodies was used. An IPAC inspection was completed to determine and mitigate the risk of blood borne infection transmission within the facilities. Appropriate protocols were followed for the IPAC investigation and public health response. The investigation identified a risk of potential HCV transmission between two cases linked to the same dental facility. There were no other epi-linked cases of HCV identified. Challenges included a lack of adherence to IPAC standards in one of the dental settings and awareness in the dental community regarding HCV transmission, coordination with regulatory bodies and public health experts and low uptake of laboratory testing by patients. Despite the unique challenges associated with the investigation, HKPRDHU conducted a successful IPAC lapse investigation and public health response. Public health units need to maintain collaborative approaches with regulated health professionals, their regulatory bodies and public health experts.

Impact of a Serious Game (Escape COVID-19) on the Intention to Change COVID-19 Control Practices Among Employees of Long-term Care Facilities: Web-Based Randomized Controlled Trial.

BackgroundMost residents of long-term care facilities (LTCFs) are at high risk of complications and death following SARS-CoV-2 infection. In these facilities, viral transmission can be facilitated by shortages of human and material resources, which can lead to suboptimal application of infection prevention and control (IPC) procedures. To improve the dissemination of COVID-19 IPC guidelines, we developed a serious game called “Escape COVID-19” using Nicholson’s RECIPE for meaningful gamification, as engaging serious games have the potential to induce behavioral change.ObjectiveAs the probability of executing an action is strongly linked to the intention of performing it, the objective of this study was to determine whether LTCF employees were willing to change their IPC practices after playing “Escape COVID-19.”MethodsThis was a web-based, triple-blind, randomized controlled trial, which took place between November 5 and December 4, 2020. The health authorities of Geneva, Switzerland, asked the managers of all LTCFs under their jurisdiction to forward information regarding the study to all their employees, regardless of professional status. Participants were unaware that they would be randomly allocated to one of two different study paths upon registration. In the control group, participants filled in a first questionnaire designed to gather demographic data and assess baseline knowledge before accessing regular online IPC guidelines. They then answered a second questionnaire, which assessed their willingness to change their IPC practices and identified the reasons underlying their decision. They were then granted access to the serious game. Conversely, the serious game group played “Escape COVID-19” after answering the first questionnaire but before answering the second one. This group accessed the control material after answering the second set of questions. There was no time limit. The primary outcome was the proportion of LTCF employees willing to change their IPC practices. Secondary outcomes included the factors underlying participants’ decisions, the domains these changes would affect, changes in the use of protective equipment items, and attrition at each stage of the study.ResultsA total of 295 answer sets were analyzed. Willingness to change behavior was higher in the serious game group (82% [119/145] versus 56% [84/150]; P<.001), with an odds ratio of 3.86 (95% CI 2.18-6.81; P<.001) after adjusting for professional category and baseline knowledge, using a mixed effects logistic regression model with LTCF as a random effect. For more than two-thirds (142/203) of the participants, the feeling of playing an important role against the epidemic was the most important factor explaining their willingness to change behavior. Most of the participants unwilling to change their behavior answered that they were already applying all the guidelines.ConclusionsThe serious game “Escape COVID-19” was more successful than standard IPC material in convincing LTCF employees to adopt COVID-19–safe IPC behavior.International Registered Report Identifier (IRRID)RR2-10.2196/25595

Education and training in infection prevention and control: Exploring support for national standards.

BackgroundEffective infection prevention and control (IPC) programmes comprise a hierarchy of preventive measures, one of which is appropriate use of personal protective equipment (PPE). A poor understanding of the role of PPE and sub-optimal use may fail to prevent or even increase pathogen transmission during routine care or an infectious disease outbreak. Variability in delivery and content of IPC and PPE education and training across organisations can lead to confusion, unsafe practice, and lack of confidence among clinicians. In a national survey we explored the perspectives of Australian and New Zealand IPC professionals on the value and feasibility of a national IPC training and monitoring programme to improve and standardise PPE practice and raise the profile of IPC.MethodsA population-based online survey that examined hospital PPE training programmes was distributed to members of three major Australasian organisations representing IPC professionals. Quantitative results of the survey have been reported previously. This paper is a qualitative analysis of responses to two open-ended questions about a national approach to training in IPC and the use of PPE.ResultsMost respondents agreed that standardising IPC and PPE training could achieve more consistent practice nationally, supported through the provision of educational resources. Including competency in the use of PPE in mandatory IPC standards would assist in improving the practice and raising the profile of IPC more generally.ConclusionThe results of this study suggest that that there is support for national programmes and standards for use of PPE in Australia and New Zealand.

Risk Factors Associated with COVID-19 Infected Healthcare Workers in Muscat Governorate, Oman.

Introduction:Coronavirus disease 2019 (COVID-19) has spread rapidly worldwide, causing a global public health crisis. Healthcare workers (HCWs) are vulnerable due to their role in the management of COVID-19 infected patients. As of June 2020, a total of 847 HCWs in Oman had reportedly contracted COVID-19, with an incidence rate of 1.47%. This study therefore aimed to identify factors associated with COVID-19 infection among HCWs in Muscat Governorate, Oman, as well as to evaluate adherence to infection prevention and control (IPC) measures.Methods:This cross-sectional study involved cases of laboratory-confirmed COVID-19 infection among HCWs working under the Directorate General of Health Services of Muscat Governorate, Ministry of Health, between February and June 2020. Data regarding the participants’ sociodemographic characteristics, risk factors, pre-existing medical conditions, and adherence to IPC measures were collected using a self-administered questionnaire distributed via a web-based mobile application.Results:A total of 126 HCWs with confirmed COVID-19 infection participated in the study. Of these, 72.2% were female, 53.2% worked in primary care facilities, and 61.1% were medical doctors or nurses. Only 18.1% were over 45 years of age and 30.2% had pre-existing medical conditions. While 29.4% had never received IPC training, the majority followed recommended hand hygiene practice (96.8%) and social distancing protocols (93.7%) and wore protective facemasks for routine patient care (96.9%).Conclusion:While the majority of HCWs followed crucial IPC measures, one-third had never received specific IPC training or faced restrictions on PPE use. HCWs, including those in housekeeping and administrative functions are recommended to undergo rigorous IPC training. In addition, high-risk HCWs could be assigned duties away from active COVID 19 cases. It is recommended to restructure health facilities for better adherence to IPC standards.

Increase in Hospital-Acquired Carbapenem-Resistant Acinetobacter baumannii Infection and Colonization in an Acute Care Hospital During a Surge in COVID-19 Admissions - New Jersey, February-July 2020.

Carbapenem-resistant Acinetobacter baumannii (CRAB), an opportunistic pathogen primarily associated with hospital-acquired infections, is an urgent public health threat (1). In health care facilities, CRAB readily contaminates the patient care environment and health care providers' hands, survives for extended periods on dry surfaces, and can be spread by asymptomatically colonized persons; these factors make CRAB outbreaks in acute care hospitals difficult to control (2,3). On May 28, 2020, a New Jersey hospital (hospital A) reported a cluster of CRAB infections during a surge in patients hospitalized with coronavirus disease 2019 (COVID-19). Hospital A and the New Jersey Department of Health (NJDOH) conducted an investigation, and identified 34 patients with hospital-acquired multidrug-resistant CRAB infection or colonization during February-July 2020, including 21 (62%) who were admitted to two intensive care units (ICUs) dedicated to caring for COVID-19 patients. In late March, increasing COVID-19-related hospitalizations led to shortages in personnel, personal protective equipment (PPE), and medical equipment, resulting in changes to conventional infection prevention and control (IPC) practices. In late May, hospital A resumed normal operations, including standard IPC measures, as COVID-19 hospitalizations decreased, lessening the impact of personnel and supply chain shortages on hospital functions. CRAB cases subsequently returned to a pre-COVID-19 baseline of none to two cases monthly. The occurrence of this cluster underscores the potential for multidrug-resistant organisms (MDROs) to spread during events when standard hospital practices might be disrupted; conventional IPC strategies should be reinstated as soon as capacity and resources allow.

Perspectives and practices of healthcare providers and caregivers on healthcare-associated infections in the neonatal intensive care units of two hospitals in Ghana.

Healthcare-associated infections (HAIs) remain a serious threat to patient safety worldwide, particularly in low- and middle-income countries. Reducing the burden of HAIs through the observation and enforcement of infection prevention and control (IPC) practices remains a priority. Despite growing emphasis on HAI prevention in low- and middle-income countries, limited evidence is available to improve IPC practices to reduce HAIs. This study examined the perspectives of healthcare providers (HPs) and mothers in the neonatal intensive care unit on HAIs and determined the major barriers and facilitators to promoting standard IPC practices. This study draws on data from an ethnographic study using 38 in-depth interviews, four focus group discussions and participant observation conducted among HPs and mothers in neonatal intensive care units of a secondary- and tertiary-level hospital in Ghana. The qualitative data were analysed using a grounded theory approach, and NVivo 12 to facilitate coding. HPs and mothers demonstrated a modest level of understanding about HAIs. Personal, interpersonal, community, organizational and policy-level factors interacted in complex ways to influence IPC practices. HPs sometimes considered HAI concerns to be secondary in the face of a heavy clinical workload, a lack of structured systems and the quest to protect professional authority. The positive attitudes of some HPs, and peer interactions promoted standard IPC practices. Mothers expressed interest in participation in IPC activities. It however requires systematic efforts by HPs to partner with mothers in IPC. Training and capacity building of HPs, provision of adequate resources and improving communication between HPs and mothers were recommended to improve standard IPC practices. We conclude that there is a need for institutionalizing IPC policies and strengthening strategies that acknowledge and value mothers’ roles as caregivers and partners in IPC. To ensure this, HPs should be better equipped to prioritize communication and collaboration with mothers to reduce the burden of HAIs.

Infection Prevention and Control Measures at the Children Hospital Lahore: A My Child Matters Collaborative Project.

PURPOSEInfection prevention among children with cancer is a major challenge at Children Hospital Lahore (CHL), a public health care facility in Pakistan with 1,000 new pediatric cancer admissions annually. The objective has been to reduce infections through collaboration between CHL and the St Jude Children’s Hospital Global Infectious Disease program via a grant by the Sanofi Espoir foundation through the My Child Matters program. The aim of the current study was to describe the effect of the collaborative improvement strategy on existing infection prevention and control (IPC) standards at CHL.MATERIALS AND METHODSOur work was a prospective before-and-after study to improve IPC standards. We compared the WHO Hand Hygiene Self-Assessment Framework and four modules of the St Jude modified Infection Control Assessment Tool (ICAT) scores over a 3-year period. Our strategy included creating a multidisciplinary team of pediatric oncologists, infectious disease physicians, nurses, a microbiologist, and a data manager; engaging in monthly online IPC mentoring sessions with St Jude Children’s Hospital Global Infectious Disease program and My Child Matters mentors; performing daily inpatient health care–associated infection surveillance rounds; and performing regular hand hygiene training and compliance audits.RESULTSBaseline needs assessment showed health care–associated infections identified by positive blood cultures as 8.7 infections per 1,000 patient-days. Deficient hand hygiene supplies, health education measures, and bed sharing of neutropenic patients were identified as major challenges. Our hand hygiene facility level, per WHO scores, increased from Inadequate to Intermediate/Consolidation by the end of the 3-year implementation (122 v 352 WHO Hand Hygiene Self-Assessment Framework scores). The sink:bed and hand sanitizer:bed ratios improved to 1:6 and 1:1, respectively. The ICAT general infection control module increased by 40% (45 v 78 ICAT scores) and hygiene compliance improved by 20%.CONCLUSIONImplementing a collaborative improvement strategy improved IPC standards in our center, which can be easily replicated in other pediatric oncology centers in lower- and middle-income countries.

Infection Prevention and Control Capacity Building During 2018–2019 Democratic Republic of Congo Ebola Virus Disease Outbreak

Background: As of July 1, 2019, ~18% of all cases in the Ebola virus disease (EVD) outbreak in the Democratic Republic of Congo (DRC) were healthcare-associated (ie, nosocomial) infections (HAIs) and healthcare worker (HCW) infections. Although progress has been achieved, gaps remained in infection prevention and control (IPC), specifically, a need to reinforce standardized, evidence-based IPC practices to effectively address HAIs. The Ministry of Health (MOH), in collaboration with partners, developed an IPC tool kit consisting of &gt;70 documents (ie, terms of reference, standard operating procedures, training modules, etc) to improve HCW IPC knowledge and practices at healthcare facilities among staff. The tool kit incorporated international IPC standards, DRC-specific experiences, and best practices. Thus, it serves as a technical and operational package, covering general guidance (standard precautions) and EVD specific issues. Methods: A decentralized rollout approach was used to disseminate the tool kit content at the various health-system levels over several months. Initially, national-level training of trainers was held, followed by subnational-level training of IPC supervisors and key IPC implementers, and lastly, training of healthcare facility (HCF) IPC focal persons. The 5-day training adhered to the MOH standard of 60% theory and 40% practice. Participants completed evaluations before and after training; changes in knowledge between the pre- and posttraining tests were analyzed and the results of the statistical tests were reported (P &lt; .05 was considered statistically significant). Results: In total, 294 IPC supervisors were trained across 7 subnational commissions. Data were analyzed for 138 participants. Participants were 60.9% IPC supervisors, 8% WASH supervisors, and 31% others. MOH representation was 52.9% The average results before the test were 66% (19.8 of 30), the average posttest results were 72% (21.6 of 30)—a significant improvement. The worst-performing pretest IPC domain was IPC approach, and facility closure was the worst performing for posttest. As of November 11, 15.7% of all cases were HAIs. Conclusions: The IPC training program initiated during an outbreak can increase knowledge and potentially improve practices and confidence. An association with the downward HAI trend is yet to be validated. The MOH anticipates that this tool kit will be the go-to resource for future Ebola outbreaks and that it will be incorporated into the preservice medical curriculum to ensure a resilient heath system.Funding: NoneDisclosures: None

Utilizing a Risk-Based Criteria Framework to Identify Infection Prevention and Control Risks in Australian Dental Settings

Background: A dental practitioner must comply with the Dental Board of Australia’s guidelines on infection control. In this project, we developed a risk-based criteria framework to assess a practitioner’s infection prevention and control (IPC) systems and processes. This project allowed for the provision of the highest standard of infection control continuing education and advice relevant to the needs of members of the Australian Dental Association (NSW Branch). Methods: A review of 1,050 continuing professional development (CPD) IPC course evaluation forms was conducted to determine the key IPC areas that participants have the most difficulty with. All core IPC documents that practitioners are expected to understand and comply with were determined and any regulator- and profession-led compliance checklists were collated. These data were consolidated to generate a risk-based criteria framework that was then applied to 99 private, office-based, dental practices to determine IPC compliance. Results: After the review of 99 dental practices, the total aggregate compliance score was 78%, and the 15 key IPC areas were ranked from highest to lowest. These data assisted with the development of a full-day IPC course focusing on the top 5 risks in each category. The five areas of opportunity identified were Hand Hygiene (52%), Surgical Procedures and Aseptic Technique (59%), Documentation, Policy and Knowledge (61%), Sharps (72%), Steam Sterilisers (72%). Conclusions: This project identified key IPC risks for office-based dental practices from the capture of performance-based data. This data formed a targeted education framework that prioritized areas of opportunity to improve IPC standards in Australian dental practices.Funding: NoneDisclosures: None

Resource Constraint Hindering Infection Prevention and Control: Evidence From Tertiary-Care Public Hospitals in Bangladesh

Background: Despite gradual economic growth in Bangladesh, healthcare facilities are still resource limited and do not measure up to the standard to practice for infection prevention and control (IPC) in reducing the risk of hospital-acquired infections. We assessed the existing resources and facilities in tertiary-care public hospitals to guide a context-specific low-resource IPC intervention for routine use in Hospital wards. Methods: We conducted the study in 3 tertiary-care hospitals from November 2017 to January 2018. The study team collected data on existing facilities and resources associated with IPC strategy from hospital records, semi-structured interviews with different level healthcare staff (n = 176) and spot checks (n = 30). Results: The mean bed occupancy rates for study hospitals were 165%, 200%, and 150%, respectively, on admission days. Among study wards, medicine ward (230%) had the highest bed occupancy rate. Different types of patients were placed together in the wards, and there were no isolation areas for highly infectious patients. Moreover, 22%–58% posts of physicians, 15%–20% of nurses and 38%–42% of support staff were vacant against the authorized posts in these hospitals. There were no usable handwashing facilities for support staff, patients, and family caregivers; however, all the allocated handwashing facilities for physicians and nurses were functional. On average, 50% fewer surgical gloves were provided than were actually required. Although supplies of masks were available in the surgery theater, no supplies in general wards were recorded. Disposable nasal cannula for oxygen and nebulizer masks was unavailable; hence, providers had to reuse this equipment for multiple patients. Most of the autoclave machines (20 of 30) were nonfunctional; therefore, one-third of the surgical instruments could not be sterilized by autoclaving. None of the hospital wards followed the 3 steps of surface cleaning, and no segregation of hazardous wastes was observed. All kind of wastes were dumped in the selected open area within the hospital premises. Healthcare workers (n = 110) directly involved in patient care reported that hand hygiene is usually not possible between patient visits. High turnover of patients and shortage of healthcare staff were reported as major barriers to IPC practices, specifically hand hygiene and environmental cleaning. There was no active committee nor specific training program on IPC for healthcare staff. Conclusions: Existing resources and facilities of these hospitals did not support a standard IPC strategy. Coordination from policy level to implementation with proper allocation of resources is required to ensure a practical IPC strategy.Funding: NoneDisclosures: None
